The percentage of positive cores represents the foremost predictor of lymph node invasion (LNI) and should be included in any LNI prediction model. In the absence of prospective data supporting the role of extended Pelvic Lymph Node Dissection (ePLND) in Prostate Cancer (PCa) outcome, ePLND might be safely omitted in all patients with a nomogram-derived LNI risk <5%.
