Deze klinische beslisregel geeft aan wanneer bij patiënten met polstrauma die de Spoedeisende Hulp (SEH) bezoeken een röntgenfoto geïndiceerd is.

Toepassingsgebied:
Het Amsterdam Wrist Rule (AWR) model kan worden gebruikt als screeningsinstrument voor het aanvragen van röntgenfoto’s bij patiënten met polstrauma.

Externe model validatie:
De discriminatieve waarde van het model bleek adequaat met een AUC (c-index) van 0,81 (95%-BI: 0,77-0,85). In het validatiecohort (n=395) bereikte het model een sensitiviteit van 98% (95%-BI: 95-99%) en een specificiteit van 21% (95%-BI: 15-28%) met als afkapwaarde een voorspelde kans van 21%. De negatief voorspellende waarde bleek 90% (95%-BI: 81-99).

Impact analyse: 
In het externe validatiecohort zou bij gebruik van de AWR een röntgenfoto zijn aangevraagd voor 89,6% (354/395) in plaats van 99,5% van de patiënten: een afname van bijna 10%.
