Post-Hepatectomy Liver Failure Risk Calculator for Patients Undergoing Major Hepatectomy
 
This risk calculator was developed and validated in a multi-institutional cohort of 2,192 adult patients who underwent major liver resection from 2010 to 2020. 

Post-hepatectomy liver failure (PHLF) is defined as International Study Group of Liver Surgery (ISGLS) grade B & C liver failure (PMID: 21236455).

Model AUC varies based on the inclusion of intraoperative factors as follows:

PHLF Preoperative + Intraoperative, AUC=0.80
PHLF Preoperative, AUC=0.78

Of note, the European Association for the Study of the Liver (EASL) released clinical practice guidelines for hepatocellular carcinoma in 2018 that defined low risk of PHLF as <5%, intermediate risk as 5-30%, and high risk as >30% (PMID: 29628281). However, one should note that these are general guidelines and that in reality, risk cutoffs will differ by center.

Using the same cohort, we also developed a risk calculator to predict Comprehensive Complication Index (CCI) > 40 (https://www.evidencio.com/models/show/2790).
