The SCORE2, intended for apparently healthy patients between 40 and 69 years old.
The SCORE2-OP, intended for apparently healthy patients (including T2DM) 70 years or older.
The SCORE2-Diabetes, intended for patients with Type 2 Diabetes Mellitus, between 40 and 69 years old.

The SCORE2 algorithms include the use of risk regions to estimate cardiovascular risk, based on the regional baseline risk for the patient.

This model includes three different equations, the standard SCORE2, the SCORE2-OP and the SCORE2-Diabetes. The SCORE2 and the SCORE2-OP were developed in 2021, and the SCORE2-Diabetes was developed as an addition to the SCORE2 in 2023. The derivation studies are included in the references.

For the SCORE2 (Patients aged 40-69 without diabetes) the SCORE2 working group and ESC Cardiovascular risk collaboration used data from 45 cohorts in 13 different countries, including a total of 677 684 individuals and 30 121 CVD events to derive the equation. The resulting model was then re-scaled using country-specific CVD mortality data from the WHO and estimated fatal and non-fatal CVD incidences with age- and sex-specific data from different countries to create the different geographic risk regions. External validation was performed using data from 25 cohorts in 15 countries with 1 133 181 individuals with 43 492 CVD events.

For the SCORE2-OP (Patients aged 70 and up) the SCORE2-OP working group and ESC Cardiovascular risk collaboration used data from 28 503 individuals with 10 089 CVD events from Norway to estimate sex-specific risk models, re-scaled to risk regions using contemporary region-specific CVD event rates and risk factor levels, and then Externally validated in 338 615 individuals with 33 219 events from 6 different cohorts. However, there is limited direct external validation in High Risk and Very High Risk regions for the SCORE2-OP.

For the SCORE2-Diabetes (Patients aged 40-69 with Type 2 Diabetes Mellitus) the SCORE2-Diabetes Working Group and ESC Cardiovascular Risk Collaboration developed an extension of the SCORE2 algorithms by using participant data from four data sets comprising 229 460 Type 2 Diabetes patients with 43 706 CVD events. They introduced three extra diabetes-specific risk factors, and externally validated the algorithm in 217 036 Type 2 Diabetes patients with 38 602 CVD events. However, there is limited direct external validation in High Risk and Very High Risk regions for the SCORE2-Diabetes.
